Our visiting nurse association hospice and palliative care teams help people close out their lives with dignity, purpose and peace. While facing a terminal illness and living at home, patients receive personal care to alleviate pain and suffering, while care and support are provided to family members.
Services Provided
- A licensed medical/social worker helps the patient and family understand issues associated with the dying process.
- Home health aides visit the patient to provide personal care.
- Hospice inpatient care manages acute symptoms and crises that cannot be cared for in the home.
- A Chaplain provides pastoral care to the patient and family and bereavement services are available for thirteen months following the loss of the patient.
