A Full Spectrum of Healthcare for Neighborhoods in Need
Through three vital CHRISTUS Foundation programs, the Healthy Neighborhoods Initiative provides integrated care to impoverished families who often consider medical services an unaffordable luxury until illnesses have progressed and emergency care is required.
The hub of the Healthy Neighborhoods Initiative is CHRISTUS St. Mary’s Clinic, at Wayside Drive and I-45 South. St. Mary’s is a full service primary care clinic located in close proximity to many CHRISTUS School Clinics, as well as near routes frequented by the CHRISTUS Healthy Living Mobile Clinics. Together these CHRISTUS Foundation for HealthCare programs build on each other’s strengths for more comprehensive patient care for the most vulnerable in our communities.
CHRISTUS School Clinics are located in numerous schools, most near CHRISTUS St. Mary’s Clinic. Students with conditions such as diabetes or asthma may be referred to St. Mary’s.
CHRISTUS St. Mary’s Clinic serves as medical home for a neighborhood in need.
CHRISTUS St. Mary’s Clinic provides a number of vital healthcare services to low-income patients in an under-served Houston community near I-45 South and Wayside Drive. Each year they care for more than 10,000 patients.
CHRISTUS Healthy Living Mobile Clinics help patients, often the elderly, monitor chronic conditions such as diabetes and hypertension. On an annual basis, they serve more than 20,000 people in need.
The Mobile Clinics make repeat visits to underserved neighborhoods every few months. To increase this caregiving capacity, a second Mobile Clinic was added. Both vehicles can deliver regular follow-up care to CHRISTUS St. Mary’s patients.
